- Your current CV (please include ABR status)
- 3 letters of recommendation (addressed to Section Chief, Janie M. Lee, MD, MSc). Please be sure that one is from your residency program director.
- One-page personal statement (including reason(s) for choosing Breast Imaging as a sub-specialty, training at UW, and long-term career goals)
- USMLE scores (unofficial copy) or COMLEX equivalent, and an ECFMG certificate (for foreign applicants)

- Dates for the 2022 Radiology Fellowship Match for 2023-24 appointments are as follows:
- August 1, 2022: Programs may accept applications
- November 1, 2022: Virtual Interview Period begins
- March 23, 2022: Match opens
- April 20, 2022: Ranking opens
- May 18, 2022: Quota change deadline
- June 1, 2022: Ranking closes
- June 15, 2022: Match Day
- July 1, 2023: Training begins
